Image Based Diagnosis : Case – 9

What is the most likely diagnosis?

A. Heart Failure
B. Tuberculosis
C. Gastric Cancer
D.
Pneumonia

Answer: C. Gastric Cancer

Explanation:
The arrow is pointing to the left supraclavicular lymph node → when enlarged, this is called Virchow’s node.

Virchow’s node = Left supraclavicular lymphadenopathy. It occurs due to metastasis via the thoracic duct, which drains abdominal structures. Most classically associated with gastric carcinoma (especially stomach adenocarcinoma). Can also be seen with other intra-abdominal malignancies (pancreas, ovaries, testes).

Other options:

A.Heart Failure → causes elevated JVP, edema, not Virchow’s node.

B. Tuberculosis → cervical lymphadenitis (esp. posterior triangle) but not classical Virchow’s node.

D. Pneumonia → causes fever, cough, infiltrates, not supraclavicular lymphadenopathy.
